White spots on aquarium fish: causes, Ich and what to do

Small white spots scattered across the body, fins or gills are most commonly Ichthyophthirius multifiliis — widely known as "Ich" or white spot disease — a parasitic infection that can be serious if left unaddressed. It isn't the only possible cause, though, and a few other conditions can look similar. This guide explains what Ich actually is, what else to consider, and what to check and do — it isn't a diagnosis for your specific fish, and treatment decisions are best made alongside a specialist for anything beyond the general guidance here.

Keeping a saltwater or reef tank?

This guide is specifically about freshwater Ich. If your fish are in saltwater, the white spots you're seeing are more likely marine Ich — caused by a different parasite, Cryptocaryon irritans, not Ichthyophthirius multifiliis below. The two look similar, but treatment isn't interchangeable, and copper-based treatments commonly used for marine Ich are unsafe to dose into a display holding corals or invertebrates. Understanding white spots

Ichthyophthirius multifiliis is a large, single-celled ciliated protozoan parasite, and the name "white spot disease" comes directly from the visible white cysts it forms as it burrows into the skin, fins and gills of an infected fish[1][2]. It's an obligate parasite — it cannot survive without a living fish host — and it's considered one of the most significant parasitic problems in freshwater fishkeeping, affecting essentially all freshwater fish species and found worldwide in both wild and captive populations[1][3].

It has a multi-stage life cycle: the visible cyst stage on the fish, followed by a free-living reproductive stage, and then a free-swimming infective stage that seeks out a new host in the water column[1]. That free-swimming stage is why treatment generally needs to address the whole tank's water, not just the individual fish showing visible spots.

Likely causes

  • Ich (Ichthyophthirius multifiliis). By far the most common explanation for fine, evenly distributed white spots. It's easily introduced by new fish, plants, substrate, décor or equipment moved between tanks without quarantine[1].
  • Lymphocystis. A viral condition that produces larger, greyish-white, cauliflower-like nodules rather than fine dots — often mistaken for Ich at a glance but visually and behaviourally distinct on closer inspection.
  • Fungal patches. Cottony or fluffy white growths, usually on damaged tissue or existing wounds, look and behave differently from the firm, grain-like cysts of Ich.

  • Natural spotting or breeding tubercles. Some species and individuals naturally show white markings or small tubercles, particularly around spawning — worth ruling out for your specific species before assuming illness.

Stress and poor water quality don't cause Ich outbreaks on their own, but they make fish more susceptible to infection and can worsen an existing outbreak — another reason to test your water alongside checking for the parasite itself.   1. Look closely at the spots

Always check the tolerance of your species and any invertebrates present first — some treatments and elevated temperatures aren't safe for scaleless fish or invertebrates.

What to check next

  • Are other tankmates showing the same spots, or any signs of irritation such as flashing or scratching against décor?
  • Is there any respiratory distress — rapid breathing or gasping — which can indicate gill involvement and is more serious?
  • Was a new fish, plant, or piece of décor added recently without quarantine?
  • Does water quality test normal, or is an underlying issue also compounding things?
  • Are the spots changing in number or distribution over a day or two, consistent with an active infection rather than a static natural marking?

Is white spot disease (Ich) fatal?

It can be. Left untreated, Ich is capable of causing very high mortality, potentially up to 100% of affected fish, particularly when gills are heavily infested. It's a genuinely serious condition, not a cosmetic one, and is worth acting on promptly rather than waiting to see if it clears up.

Can Ich go away on its own?

It's not something to rely on. Ich has a life cycle with a free-swimming stage that continues spreading through the tank even after visible spots drop off an individual fish, so a fish that looks temporarily clear isn't necessarily free of the parasite. Treat the whole tank's water, not just the fish showing spots.

Do I need to treat the whole tank or just the affected fish?

Generally the whole tank. Because Ich has a free-swimming stage present in the water itself, isolating only the fish with visible spots typically isn't enough to stop it spreading to the rest of your livestock.

Are white spots always Ich?

No. Other conditions can look similar, including Lymphocystis (which forms cauliflower-like nodules rather than fine salt-grain-sized dots), fungal patches, velvet (a much finer, powdery dusting rather than discrete spots), and in some species natural spotting or breeding tubercles. Look closely at the size, texture and distribution of the spots before assuming a single cause.
